Formative Assessment
Assessment Summary
During this unit of Algebra I, we will be focusing on slope-intercept form. We will be working on graphing linear functions using slope-intercept form as well as deriving equations of lines by looking at graphs. Students come in with different levels of experience with these concepts, so it is important to start with the very basics so that everyone is on the same page. The assessment below will be used by me to get a gauge of what topics need to be focused on more. This will also allow me to see how much more of an improvement and understanding the students gain as I teach this unit when they complete their Summative Assessment at the end of this unit.

Assessment Directions/Grading
Students are required to take the assessment below before the unit is covered in class. They will print out the criss cross puzzle that I have created through Puzzlemaker, complete it, and turn it in at the beginning of class the next day. Students will be graded on whether they have completed it or not. This is an all-or-nothing assignment.
Summative Assessment
Assessment Summary
Students will be creating their own exam or quiz for this summative assessment for me to value how well they understand the concepts of graphing and comprehending slope intercept form. This activity will allow me to see how well the students understand this unit by creating questions that are both conceptual and situational. This can be used within the unit, for when I create a test at the end of the year, I can include some questions that students have created in their quiz/exam. Student thinking that will be assessed through this assessment will be creativity in the various questions created, understanding in the concept, and ability to create different types of questions through their understanding of slope intercept form and how to graph them. This exam will also help students think outside of the box so that questions are not too easy, but challenge their peers to solve. Below is an example quiz that I have created through Surveymonkey for students to use as a guideline.
